✔️ Pharmacology

Vitamin E functions as an antioxidant in the body, protecting polyunsaturated fatty acids (components of cellular membranes) and other oxygen-sensitive substances like vitamins A and C from oxidation. In premature neonates, a deficiency in vitamin E can cause irritability, edema, thrombosis, and hemolytic anemia. Adults and children with low serum tocopherol concentrations may experience creatinuria, ceroid deposition, muscle weakness, decreased erythrocyte survival, or increased in vitro hemolysis by oxidizing agents.

• Age-related Macular Degeneration: Vitamin E 400-600 IU daily.
• Heart Disease (to improve the effectiveness of Nitrates): Vitamin E 200 IU three times daily.

✔️ Interaction

Evit may impair the absorption and function of vitamins A and K and can potentiate the effect of Warfarin.

✔️ Contraindications

• Contraindicated in patients with known hypersensitivity to vitamin E.
• Patients with rare hereditary problems of fructose intolerance, glucose-galactose malabsorption, or sucrose-isomaltase insufficiency should not take this medicine.

✔️ Overdose Effects

Transient gastrointestinal disturbances have been reported with doses greater than 1000 IU daily. General supportive measures should be employed where necessary.

✔️ Side Effects

Nausea, Tiredness, Headache, Rash

• Diarrhea and abdominal pain may occur with doses greater than 1000 IU daily.

✔️ Pregnancy & Lactation

• There is no evidence supporting the safety of high doses of vitamin E during pregnancy, thus it should not be used, especially in the first trimester.
• No information is available on excretion in breast milk; therefore, it is advisable not to use it during lactation.

✔️ Precautions & Warnings

• Evit has been reported to increase bleeding tendency in vitamin K deficient patients or those taking anticoagulant treatments. It is therefore recommended to monitor the prothrombin time and international normalized ratio (INR) to detect any changes in hemostasis.
• Evit has been reported to increase the risk of thrombosis in patients predisposed to this condition, including patients taking estrogens. This should be considered when selecting patients for treatment, particularly women taking oral contraceptives containing estrogens.
