Tevirax is a nucleoside analog reverse transcriptase inhibitor. It works as follows:
It has minimal effect on human DNA polymerases, including mitochondrial DNA polymerase γ, so mitochondrial toxicity is unlikely.
Headache, stomach pain, fatigue, cough, nausea, back pain
Serious (requires medical attention):
Adult Dose:
Standard dose: 25 mg (1 tablet) orally once daily with food.
Renal Impairment:
No adjustment needed if creatinine clearance (CrCl) ≥15 mL/min or in patients on chronic hemodialysis.
Administer after hemodialysis on dialysis days.
Not recommended for ESRD patients not on hemodialysis.
Hepatic Impairment:
No adjustment for mild hepatic impairment (Child-Pugh A).
Not recommended for decompensated liver disease (Child-Pugh B or C).
Pediatric Use: Safety and effectiveness in patients under 18 years is not established.
Geriatric Use: No significant safety or efficacy differences observed in patients aged ≥65 years.
Important:
Test for HIV before starting Tevirax; do not use alone in patients with HIV.
Tevirax is affected by P-gp and BCRP inhibitors or inducers, which can increase or decrease drug absorption.
Drugs that affect renal function or compete for active tubular secretion may raise tenofovir levels and increase side effects. Examples include acyclovir, ganciclovir, aminoglycosides, high-dose NSAIDs, and others.
Pregnancy: Limited data show a small risk of birth defects; weigh benefits vs. risks.
Breastfeeding: Unknown if excreted in human milk. Consider breastfeeding benefits versus potential drug risks.
Hepatitis B exacerbation: Monitor closely after stopping treatment; therapy may need to resume.
HIV resistance risk: Do not use alone in patients with HBV/HIV co-infection. HIV testing is required before treatment.
Kidney monitoring: Regularly check serum creatinine, urine protein/glucose, and phosphorus (if chronic kidney disease).
Lactic acidosis/hepatotoxicity: Stop Tevirax immediately if severe symptoms develop.
