Liptor EZ 10/10 is a fixed-dose combination of Atorvastatin Calcium 10mg (statin) and Ezetimibe 10mg (cholesterol absorption inhibitor), manufactured by ACME Laboratories Ltd. — one of Bangladesh's most trusted pharmaceutical companies. Indications (Uses)

• Primary hyperlipidemia — elevated LDL cholesterol, total cholesterol, and triglycerides
• Mixed dyslipidemia — combined elevations of cholesterol and triglycerides
• Homozygous familial hypercholesterolemia (HoFH) — as adjunct or when other treatments are unavailable
• Secondary prevention of cardiovascular events in high-risk patients (post-MI, angina)
• Reduction of Apo B, TG, non-HDL-C; increase in HDL-C

Mechanism of Action (Pharmacology)

Atorvastatin competitively inhibits HMG-CoA reductase, the rate-limiting enzyme in hepatic cholesterol biosynthesis. This reduces intracellular cholesterol, upregulates LDL receptors on liver cells, and enhances LDL clearance from the bloodstream — reducing LDL-C by 30–50%.

Ezetimibe selectively inhibits the Niemann-Pick C1-Like 1 (NPC1L1) transporter at the brush border of the small intestine, reducing cholesterol and plant sterol absorption by approximately 50%. The combination reduces LDL-C by 55–65% — significantly more than Atorvastatin alone.

Dosage & Administration

• Adults: 1 tablet (Atorvastatin 10mg + Ezetimibe 10mg) once daily at any time of day, with or without food
• Higher-dose combination (Atorvastatin 20mg + Ezetimibe 10mg) available for patients requiring greater LDL reduction
• Dose adjustment based on treatment goals and LDL response after 4–6 weeks
• Liver function tests recommended before starting and periodically during treatment

Contraindications

• Active liver disease or unexplained persistent transaminase elevations
• Pregnancy (Pregnancy Category X for statins) and breastfeeding
• Known hypersensitivity to Atorvastatin, Ezetimibe, or any excipient

Side Effects

Common: Headache, myalgia (muscle pain/ache), arthralgia (joint pain), nausea, constipation, diarrhea, abdominal pain.

Serious (rare): Myopathy or rhabdomyolysis (severe muscle breakdown — seek immediate care if unexplained muscle pain or dark urine), hepatotoxicity (liver enzyme elevation), immune-mediated necrotising myopathy (IMNM).

Drug Interactions

• Increased myopathy risk: Cyclosporine, clarithromycin, itraconazole, HIV protease inhibitors, gemfibrozil and other fibrates
• Reduced absorption of Ezetimibe: Bile acid sequestrants — take Liptor EZ ≥2 hours before or ≥4 hours after cholestyramine
• Warfarin: Atorvastatin may enhance anticoagulant effect — INR monitoring recommended
• Grapefruit juice: Large quantities increase Atorvastatin plasma levels — avoid excessive intake

Storage

Store below 30°C in a cool, dry place. Protect from light and moisture. Keep out of reach of children.
