✔️ Administration

Route: Oral, always with food (preferably after a full meal)

Adult Dose (typical):

• Oropharyngeal candidiasis: 100 mg daily × 15 days
• Vulvovaginal candidiasis: 200 mg twice daily × 1 day
• Pityriasis versicolor: 200 mg daily × 7 days
• Tinea corporis/cruris: 100 mg daily × 15 days
• Onychomycosis: 200 mg daily × 3 months (or pulse dosing as prescribed)
• Systemic infections: 100–200 mg once/twice daily, depending on severity

Pediatric Dose: 5–10 mg/kg/day (max 200 mg/day); safety not established <3 years

Elderly: Use with caution, dose adjustment may be required

✔️ Side Effects

Common (>10%):

Nausea, abdominal pain, headache

Occasional (1–10%):

Rash, vomiting, edema, diarrhea, fever, pruritus, fatigue, dizziness, hypertension, abnormal LFTs

Rare (<1% but serious):

Hepatitis, decreased libido, hypertriglyceridemia, hypokalemia

✔️ Drug Interactions

Increased plasma concentration: anticoagulants (warfarin), digoxin, corticosteroids, alprazolam, midazolam, HIV protease inhibitors, macrolide antibiotics

Reduced levels with: rifampicin, phenytoin, carbamazepine, isoniazid, phenobarbital

Dangerous interactions:

• QT-prolonging agents (cisapride, quinidine, terfenadine, etc.) → risk of arrhythmia/torsades de pointes
• Statins (simvastatin, lovastatin, atorvastatin) → risk of rhabdomyolysis
• Fentanyl → risk of fatal respiratory depression
• Ergot alkaloids → risk of ergotism

✔️ Contraindications

• Hypersensitivity to itraconazole or other azole antifungals
• Pregnancy and lactation (unless benefits outweigh risks)
• Severe hepatic disease
• Congestive heart failure (CHF) history or active CHF
• Renal impairment (IV use contraindicated if CrCl <30 ml/min)

✔️ Precautions

• Use cautiously in patients with renal/hepatic impairment, COPD, or cardiac disorders
• Monitor liver function tests in prolonged use (>1 month)
• May impair alertness (dizziness, blurred vision) → caution with driving/machinery
• Avoid use in pregnant and breastfeeding women unless strictly necessary
• Potentially fatal: Liver failure, congestive heart failure, pulmonary edema, cardiovascular disease
