Itraconazole is a broad-spectrum azole antifungal used to treat, control, and prevent fungal infections. It acts by inhibiting ergosterol synthesis, an essential component of fungal cell membranes, thereby stopping fungal growth.
It is effective in both superficial and systemic mycoses and is prescribed for use in immunocompetent and immunocompromised patients.

Systemic Mycoses:

  • Blastomycosis
  • Histoplasmosis (non-meningeal)

  • Coccidioidomycosis

Superficial Mycoses:

  • Onychomycosis (fingernail/toenail fungal infections)
  • Dermatophytoses (tinea capitis, tinea corporis, tinea cruris, tinea pedis)
  • Pityriasis versicolor
  • Sebopsoriasis
  • Vulvovaginal and chronic mucocutaneous candidiasis

Common: Nausea, vomiting, abdominal pain, diarrhea, headache, dizziness, blurred vision

Medicines that decrease Itraconazole levels:

  • Carbamazepine, rifampin, rifabutin, phenytoin, phenobarbital, didanosine, and isoniazid
  • Acid suppressants (antacids, H2-blockers like ranitidine/cimetidine, PPIs like omeprazole, sucralfate) → reduce absorption

Medicines that increase risk of toxicity (CYP3A4 inhibition):

  • Statins (lovastatin, simvastatin, atorvastatin) → risk of rhabdomyolysis
  • Cyclosporine, tacrolimus → increased serum levels
  • Digoxin, phenytoin, warfarin → increased plasma concentration
  • Protease inhibitors, benzodiazepines, corticosteroids, oral hypoglycemics, calcium channel blockers (e.g., amlodipine) → increased effects
  • Astemizole, cisapride, terfenadine, vinca alkaloids → risk of QT prolongation, torsades de pointes, and cardiotoxicity (concomitant use contraindicated)

Liver disease: Monitor LFTs closely; discontinue if hepatotoxicity develops

Heart disease: Risk of congestive heart failure, arrhythmia, or torsade de pointes – avoid in CHF patients

Renal impairment: Use with caution, dose adjustment required

Lung disease: Inform the physician before use

  • Can cause dizziness or blurred vision → avoid driving or hazardous work until response is known
