✔️ Pharmacology

Amitriptyline: A tricyclic antidepressant that works by inhibiting the reuptake of norepinephrine and serotonin in the brain, which enhances their availability and produces an antidepressant effect.

Chlordiazepoxide: A benzodiazepine that enhances GABA-mediated chloride influx through GABA receptor channels, leading to membrane hyperpolarization. This produces sedative and anxiolytic effects.

✔️ Overdose Effects

• Amitriptyline overdose: cardiac arrhythmias, severe hypotension, convulsions, CNS depression, coma.
• Chlordiazepoxide overdose: somnolence, confusion, coma, diminished reflexes.
• Withdrawal: abrupt discontinuation may cause symptoms ranging from mild (dysphoria, insomnia, headache, malaise) to severe (convulsions, tremor, cramps, vomiting, sweating).

✔️ Dosage & Administration

Dosage must be individualized based on symptom severity and patient response.

Once improvement is achieved, the dose should be reduced to the lowest effective maintenance level.

The larger part of the daily dose may be taken at bedtime; in some cases, a single bedtime dose is sufficient.

Typical initial dosage: 3–4 tablets daily in divided doses, or as directed by the physician.

✔️ Drug Interactions

Due to its Amitriptyline component, Wella may block the antihypertensive effect of guanethidine and similar drugs.

✔️ Contraindications

Hypersensitivity to benzodiazepines or tricyclic antidepressants

Concomitant use with monoamine oxidase inhibitors (MAOIs)

✔️ Pregnancy & Lactation

Safety during pregnancy and lactation has not been established.

Should only be used if clearly needed and prescribed by a physician.

✔️ Use cautiously in patients with:

• History of seizures
• Hyperthyroidism or those on thyroid medication
• Renal or hepatic impairment
• Pediatric patients on antidepressants must be closely monitored for suicidality, behavioral changes, or clinical worsening.
• Elderly or debilitated patients: dosage should be minimized to reduce risks of ataxia, oversedation, confusion, or anticholinergic effects.
