Gefapix is prescribed for adult patients suffering from either refractory chronic cough or unexplained chronic cough when other underlying causes have been ruled out.

Gefapixant is a selective P2X3 receptor antagonist, which targets ATP-sensitive ion channels located on C-fibers of the vagus nerve in the respiratory tract. These fibers are activated during inflammation or chemical irritation, leading to the release of ATP, which triggers a cough reflex. By inhibiting ATP from binding to these receptors, Gefapixant reduces the cough signal and alleviates chronic coughing.

Common: Upper respiratory infections, nausea, diarrhea, appetite loss, dizziness, dry mouth, oral numbness, increased salivation, stomach discomfort, and indigestion.
Occasional: Urinary stones, nephrolithiasis.

✔️ Dosage & Administration

Adults: 45 mg tablet orally twice daily, with or without food.

Elderly: No dose adjustment is needed, but start cautiously due to possible reduced kidney function.

Pediatrics: Not recommended for use in patients under 18 years.

Renal Impairment:

Mild/Moderate (eGFR >30 mL/min/1.73m²): No adjustment needed.

Severe (eGFR <30 mL/min/1.73m², non-dialysis): Reduce to 45 mg once daily.

Hepatic Impairment: Not studied; caution is advised.

Obstructive Sleep Apnea: Not recommended for patients with chronic cough and concurrent sleep apnea.

Missed dose: If missed, skip and resume at the next scheduled dose.

Omeprazole (PPI): No significant effect on Gefapixant pharmacokinetics.

Gefapixant has low potential to interact with cytochrome P450 enzymes.
