✔️ Adult Dosage of Folinic 5
Oral:
- Methotrexate Toxicity Antidote:
- 15 mg every 6 hours for 10 doses, starting 24 hours after methotrexate infusion.
- Continue until serum methotrexate levels are <0.05 micromolar.
✔️ Pharmacology
- Enters cells as 5-methyl tetrahydrofolate, supplying the cofactor blocked by methotrexate.
- Stabilizes binding of 5-dUMP and thymidylate synthetase, enhancing fluorouracil activity.
- Neutralizes toxic effects of folic acid antagonists like methotrexate but enhances the effects of fluorouracil.

- Allergic reactions: Rash, pruritus, erythema, urticaria.
- Nausea, vomiting, fever (pyrexia).
✔️ Drug Interactions
- Fluorouracil:
- Folinic acid can increase fluorouracil toxicity, particularly in elderly patients, causing:
- Severe enterocolitis
- Diarrhea
- Dehydration
- Folinic acid can increase fluorouracil toxicity, particularly in elderly patients, causing:
- Sulfamethoxazole-Trimethoprim:
- Folinic acid may reduce the efficacy of sulfamethoxazole-trimethoprim in treating Pneumocystis carinii pneumonia in AIDS patients.
✔️ Precautions
- Be cautious with undiagnosed megaloblastic anemia and folate-dependent tumors.
- During combined 5-fluorouracil/Folinic acid therapy:
- Monitor calcium levels.
- Administer parenterally if gastrointestinal toxicity, nausea, or vomiting is present.
- Monitoring Required:
- Serum methotrexate levels to adjust folinic acid dosage and duration.
- Regular CBC, electrolytes, and liver function tests before and during treatment.

- Hypersensitivity to folinic acid or any components.
- Pernicious Anaemia and other megaloblastic anemias due to vitamin B12 deficiency.
- Intrathecal and Intraventricular Administration: Not recommended.
