✔️ Dosage & Administration: Deflazit 24

Adults:

• Acute conditions: Up to 120 mg/day may be required initially. Maintenance doses generally range from 3-18 mg/day.
• Rheumatoid arthritis: 3-18 mg/day; use the lowest effective dose and increase if necessary.
• Asthma: Acute attacks may need 48-72 mg/day, gradually reduced after symptom control. Maintenance doses should be adjusted to the lowest effective dose.
• Other conditions: Dose should be tailored to clinical needs, aiming for the lowest effective dose. Typically, 6 mg of Deflazacort equals 5 mg of prednisone or prednisolone.

Children: 

Limited data exists for children, but the same indications apply. The recommended dose ranges from 0.25-1.5 mg/kg/day, with alternate-day administration possibly appropriate.

• Juvenile chronic arthritis: 0.25-1.0 mg/kg/day
• Nephrotic syndrome: 1.5 mg/kg/day, taper as needed
• Asthma: 0.25-1.0 mg/kg on alternate days

• Liver impairment: Monitor closely and adjust dose as blood levels may increase.
• Kidney impairment and elderly patients: Follow standard precautions for corticosteroid use. Elderly patients may experience more severe adverse effects.

Withdrawal: For patients on more than 9 mg/day of corticosteroids for over three weeks, gradual dose reduction is necessary to prevent disease relapse.

✔️ Drug Interactions:

Deflazit is metabolized in the liver. Dose adjustments may be required when co-administered with:

• Liver enzyme inducers like rifampicin, rifabutin, carbamazepine, phenobarbitone, phenytoin, primidone, and aminoglutethimide (may require an increased Deflazit dose).
• Liver enzyme inhibitors like ketoconazole (may require a reduced Deflazit dose).

✔️ Deflazit should not be used in patients who:

• Are allergic to any ingredients in the drug
• Are receiving live virus immunizations

✔️ Pregnancy & Lactation:

• Pregnancy: Deflazacort crosses the placenta. Long-term or repeated use during pregnancy may increase the risk of intrauterine growth retardation. It should be prescribed only when the benefits outweigh the risks.
• Breastfeeding: Corticosteroids are excreted in breast milk. Daily doses up to 50 mg are unlikely to affect the infant significantly. Higher doses may suppress the infant's adrenal function, but breastfeeding benefits typically outweigh the risks.

• Cardiac disease or heart failure (excluding active rheumatic carditis)
• High blood pressure
• Blood clotting disorders
• Gastrointestinal issues like gastritis, peptic ulcers, or risk of intestinal perforation
• Diabetes or family history of diabetes
• Osteoporosis
• Muscle weakness disorders like myasthenia gravis
• Kidney insufficiency
• Mental health issues such as emotional instability or psychosis
• Epilepsy
• Previous corticosteroid-induced muscle weakness
• Liver failure, hypothyroidism, or cirrhosis
• Ocular herpes simplex (risk of corneal perforation)
