✔️ Dosage & Administration

Adults

• 200–400 mg daily, as a single dose or divided into two doses
• Gonorrhea: 400 mg as a single dose
• Typhoid fever: 20 mg/kg daily in 2 divided doses for 7–14 days

Children

• Above 6 months: 8 mg/kg/day in 1–2 divided doses
• 6 months–1 year: 75 mg daily
• 1–4 years: 100 mg daily
• 5–10 years: 200 mg daily
• >12 years or >50 kg: Adult dose

Typhoid fever: 15–20 mg/kg/day in 2 divided doses for 7–14 days (max 400 mg/day)

<6 months: Safety not established

Renal Impairment

If CrCl <20 mL/min → maximum dose: 200 mg/day

Administration

Can be taken with or without food

Best taken with food or milk to minimize stomach upset

✔️ Side Effects

• Nausea, abdominal pain, diarrhea, indigestion
• Headache, dizziness, flatulence, rash, pruritus, candidiasis
• Elevated liver enzymes, eosinophilia, leukopenia
• Pseudomembranous colitis (can be fatal)
• Stevens-Johnson syndrome
• Serum sickness-like reactions
• Thrombocytopenia
• Hypersensitivity reactions (rash, itching, facial swelling, breathing difficulty)

✔️ Interaction

Probenecid: Increases cefixime concentration

Anticoagulants: May increase prothrombin time → bleeding risk

Additive effects are possible with other nephrotoxic drugs

✔️ Contraindications

Hypersensitivity to cephalosporins

Use with caution in patients with a history of penicillin allergy

Pregnancy: Studies with cephalosporins, including cefixime, show no confirmed risk of major birth defects or miscarriage. However, use only if clearly needed.

Lactation: Drug presence in human milk not well studied; caution advised. Likely excreted in small amounts, as with other β-lactams.
